How many times the submitted paper to any math journal stay in " decision in process" step ?

I would be interest like some one to show me that " How many times that the submitted paper to any mathematical journal stay in " decision in process" step after under review step ? and i want also to know if the paper passed from " under review " to " decision in process" step without requerment of revision , does it mean that the paper will be reject ?
