Who do I report a professor to that is stacking the deck for students to do poorly?

I had a math professor recently do the following:

1. 
   

   Failed to provide an answer key to a test until 2 days before the test.

   
   


2. 
   

   Had at least 5 of the answers wrong on the practice test answer key, which he then made a joke about before the test.

   
   


3. 
   

   Had the students learn an entirely new problem set that they did not study for, or go over in the material they learned the week before the test.

   
   


4. 
   

   Weighted each of the questions on the test differently.

   
   


5. 
   

   Distributed 100 points across a 13-problem test.

   
   

I strongly feel that he is setting the students up to fail instead of providing them with adequate instruction so that they succeed.
