Including an index of notations in a mathematical Ph.D. thesis

I have asked a previous question and in one of the answers, it was suggested that I include a "cheat sheet" table up front that gives the definitions of all important symbols or symbol-classes.

Now I would like to ask: Is including this sheet is a good idea for a mathematical Ph.D. thesis? I have only seen this sheet in books, but never theses or papers.

If it is a good idea, I would like to ask:

1. Where in my thesis should I include it?


2. Is there any standard form for it? (Will appreciate if anyone could provide a LaTeX template, although asking a LaTeX question may be off topic here.)
