Medical fitness certificate format for foreign university

I am applying to a foreign university for scholarship and for that purpose the university is asking for a medical fitness certificate. The problem is that the university doesn't provide any kind of format for Medical fitness certificate. When i visited my doctor; as i am applying for foreign university he recommended some kind of format. So my question is for applying to foreign university is their any kind of international recognized medical certificate format or else can i have a simple letter in the form of doctors letter head stating my fitness of health. If possible can you also put the link of some medical format.
